Welcome to Jazz Youth BINGO!!! Here’s how it works …

• Download and print out your BINGO board, and then complete it at your own pace.
• When you get a BINGO, simply fill in the form below and you’ll receive a link to a prize.
• You can win prizes for up to four BINGOs.
• If you get a BLACKOUT, you’ll receive a $5 credit to the Utah Jazz Team Store PLUS you’ll be entered to win 1 of 5 grand prizes.
• You have until May 15 to complete the board.
• Ages 4–18.
